Steps 1,2,3 of the registration process below must be completed before the first day of practice.  If needed, contact Coach Ian at 796-1333 x318 or coachcountryside@hotmail.com.

1) Complete the Team Registration Form.

2) Complete the Florida High School Athletic Association (FHSAA) Registration Packet.  
     Note: doctor physicals are required.

3) All swimmers must have School Insurance.

4) All swimmers/divers are required to purchase a team suit, cap and polo shirt.  Apparel information is available on the home page.

5) Complete fundraising forms on the home page under Fundraising. 

6) Parents: Volunteer hours are required.  Before first meet, complete the Volunteer Registration Form.

Please complete all of the forms and return to Coach Ian at the Countryside Country Club Pool.  There is a folder for the High School Team on the pool deck.
