Registration packets (contact Coach Ian if you do not have one 796-1333 x318 or email coachcountryside@hotmail.com) must be filled out before the first day of practice, August 11, 2008.

All swimmers must have school insurance (www.k12studentinsurance.com) and the Florida High School Athletic Association Consent and Release from Liability Certificate completed

Doctor physicals are required

Before volunteering at any meet the Volunteer Registration Form must be completed.

All swimmers/divers are required to purchase a team suit, cap and polo shirt.  Apparel information is available on the home page.

Please complete all of the forms and return to Coach Ian at the Countryside Country Club Pool.  There is a folder for the High School Team on the pool deck.
